Message from the Head of the Department

  • Our department is a clinical department newly established in Saitama Medical Center, Dokkyo Medical University in April 2018.
  • We are working in ICU to specialize in treating severe cases regardless of medical or surgical.
  • We have an ICU outreach team. It is organized by intensivists, nurses, and clinical engineers to provide ICU orientation to patients and their family and consultation with critically ill patients from attending doctors.
  • Recently, we are actively accepting and treating many severe patients with "COVID-19".
  • Because the "Department of Intensive Care Medicine" is not common yet in Japan, there are not many hospitals nationwide that advocate the same department
  • As an innovator, we intend to promote safe and high-quality intensive care in cooperation with each department in our institution so that many critically ill patients can fully rehabilitate.

Activities

1.Clinical work

  • Management and operation of the general ICU in Saitama Medical Center, Dokkyo Medical University
  • Being a leader of the multidisciplinary team in the ICU
  • Operation of artificial supports for severe patients with organ failure, such as mechanical ventilation, blood purification and ECLS, etc

2.Quality control

  • Practice of various clinical guidelines
  • Measurement of clinical indicators, such as VAE surveillance
  • Daily multidisciplinary conference to discuss patient issues

Research Field

  • Respiratory management for acute respiratory distress syndrome
  • Analgesia / sedation / delirium management in critically ill patients
  • Physical therapy or early mobilization for critically ill patients
  • Metabolism and nutrition of critically ill patients
  • Infection control in the ICU

Characteristics of the section

  • The management model of our ICU is called "high-intensity ICU", and it is in the form of "a doctor in each department serves as the attending physician, and the intensivists participant in the medical care of all those patients". It is known that such ICU can reduce the mortality of critically ill patients by nearly 20% compared to the patients where intensivists are not involved.
  • Recently, the number of hospitalized cases with multiple disabilities at the same time is increasing due to the increase in elderly patients.
  • We are trained to manage the whole body of critically ill patients with many problems while balancing each organ function and called “cross-organ clinical doctor”.
  • By joining the medical team, we will clear various problems, eventually withdraw from mechanical ventilation, and lead to recovery of ADL through exercise and nutritional support.
  • Intensivists work in cooperation with the attending physicians of various clinical departments, so we try to treat them based on scientific evidence rather than individual experience.
  • Currently, many medical practice guidelines have been published by various academic societies, and we also refer to these guidelines for medical care.
