Department of Obstetrics and Gynecology

Head of the Office: Akira Mitsuhashi MD, PhD

Message from the Head of the Office

Obstetrics and gynecology are specialties that cover a wide range of fields such as gynecological oncology, perinatal care, reproductive endocrinology, and women's health, and our department provides specialized medical care by specialists in each of these fields. We will fulfill our role as a medical center for the local community and provide advanced and safe medical care to local residents. On the research side, we aim to solve questions that arise in daily clinical practice and we actively conduct clinical research in addition to basic research, with an aim to establish new treatment methods.

Activities

1. Clinical Activity

  1. As the obstetrics department of the Perinatal Medical Center, we provide maternal and fetal care for high-risk pregnancies. We are a tertiary emergency facility in Tochigi prefecture, and actively accept referrals from other medical institutions and provide maternal transport. We also provide prenatal genetic testing and genetic counseling.
  2. In the field of gynecological oncology, as a regional cancer base hospital, we provide medical care in collaboration with multiple departments, including the radiology, surgery, and urology departments, among others. We actively accept patients with medical complications that are difficult to manage at other hospitals. We also perform minimally invasive robotic surgery and risk-reducing salpingo-oophorectomy for cases of hereditary breast and ovarian cancer.
  3. In addition to infertility treatment, we are also working on fertility preservation for young cancer patients.
  4. As part of caring for women's health, we treat women from adolescence to menopause and old age, with the goal of improving their quality of life.

    Target diseases:
  • Gynecologic tumors: uterine leiomyoma, ovarian tumors, cervical cancer, endometrial cancer, ovarian cancer, and other gynecological cancers
  • Women's health: adolescent/menopausal health care, and sexually transmitted diseases
  • Perinatal care: prenatal checkup, high-risk pregnancy management, and genetic consultation

    Characteristics
  • Robot-assisted laparoscopic hysterectomy for endometrial cancer and benign uterine disease.
  • Risk-reducing salpingo-oophorectomy for hereditary breast and ovarian cancer.
  • Uterine artery embolization for uterine fibroids (jointly with the Department of Radiology)
  • In vitro fertilization/intracytoplasmic sperm injection/freezing and thawing embryo transfer
  • Fetal diagnosis and therapeutic intervention in utero
  • Outpatient genetic counseling and investigation

2.Research Activity

In the field of cancer research, we are investigating individualized treatments based on the genetic expression of cancer. We are investigating new prognostic factors and carcinogenesis based on the protein/gene level, along with the effects of new therapeutic candidate drugs. We also conduct research in the perinatal period aimed at improving treatment results for imminent preterm birth, preeclampsia, fetal dysfunction, fetal growth restriction, and fetal disease.

Some of our subjects of research are:
  • Search for objective indicators to determine the adequacy of chemotherapy for patients with end-of-life ovarian cancer.
  • Biological significance of platelet aggregation factor podoplanin (D2-40) expressed in clear cell ovarian cancer.
  • Clinical study of metformin to prevent endometrial cancer by targeting obesity and insulin resistance.
  • Elucidation of the abnormal metabolic profile of endometrial cancer.
  • Study of humoral factors, including female hormones, and vascular function.
  • Research on various treatments and changes in quality of life (QOL) in women's health.
  • Measurement of fetal heart global longitudinal strain and the strain rate using 2DTT.
  • Analysis of fetal circulation using the phased-tracking method.
  • Development and clinical application of fetal treatment with a fetoscope.
